We need to let the AG know what pain I and others have everyday and the pain killers we need that he makes hard for our docs to write. So let us pray his doc will not give him the pain killers he needs until he begs for them. Then let us pray the doc tells him what he put doctors through.

That's just not right.

I don't like Ashcroft's policies, I think he's a threat to liberty, but I don't wish him to suffer, and I hope he'll recover quickly and fully.

That said....

John Ashcroft's probably in horrible pain right now, probably clutching his guts while it feels like his body's being torn apart.

It's probably almost as painful as terminal cancer.

So I want to help relieve his suffering. It just isn't right to let anybody suffer like that, if you can help them, no matter what ideologies are involved.

So I tried to call my friend Tommy, to see if I could get John some natural chronic pain-reliever, but they told me the Feds done busted ol' Tommy Chong.

So then I tried calling one of them mary-jane co-operatives in Cali, man, for people suffering in terrible pain. But they told me the Feds had shut them down too, even though they're legal under California law.

But then, man, I was like, I know this doctor in Oregon, and they have real progressive laws about pain relief for the terminally il. But the doctor I talked to told me that even though death with dignity is legal in Oregon, the Feds told the doctors that if they helped those poor people, they'd lose their Federal drug licenses no matter what Oregon law is.

So it's a real bummer. I'd like to help John Ashcroft and all those other people who live, day after miserable day in terrible, debilitating, humiliating pain, but even though that's legal in some states, the Feds say they have to suffer.

I wonder who tells the Feds to do that?

Maybe when John gets out of the hospital, he'll empathize with all those people who can't get marijuana to palliate their glaucoma, their AIDS, their incredibly painful terminal cancers, people who, like Christ, been doing the suffering for John's sins.

Well I expect he would figure "Do as you would be done by" and that bastard has caused my loved one to suffer agony such as that I prayed for God to take him to end his suffering. Wait and see people! you who haven't suffered extreme pain or seen someone you love who cries though he was a stalwart grown man, wait a few years---you'll run into that circumstance and you will wonder why you could have been so blind.

There was a story that Ashcroft was planning to visit England, and he requested that any calico cats should be removed from where he would be going. Why? Well, for chromosomal reasons, calico cats are almost always female, and male calico cats are almost always sterile (they tend to have one X chromosome too many). It it any wonder that Christian fundies would associate them with withcraft and satanism?

She died within 2 months from that horrible disease. She couldn't eat and it was painful.

That man disgusts me, but I wouldn't want anyone to suffer the way she did. Wish they could find some damn cures for these things...oh yeah, that would be STEM CELL RESEARCH. Something I sure he's all for now.

179. I had pancreatitis once -
a doctor acquaintance saw me in the elevator and when I told him why I was in the hospital - he said "now you know what real pain is." I couldn't agree more. I can't imagine anything worse - many times worse than childbirth. Morphine doesn't help much, but allows you to sleep for an hour or so. I wouldn't wish it on anybody.

Biliary tract disease (gallstones) is the most common cause of acute pancreatitis in non-alcoholic Americans, and is the leading cause worldwide. Other etiologies include penetrating peptic ulcer, trauma, post-ERCP, post-operative, metabolic (hypertriglyceridemia), and drug-induced. Presenting symptoms include severe constant abdominal pain, nausea, emesis, fever and tachycardia.

In approximately 85 to 90% of patients, acute pancreatitis is self-limited and resolves completely in 3 to 7 days after treatment is instituted. Medical therapy is aimed at reducing pancreatic secretion, thereby "resting" the pancreas, and usually involves analgesia, intravenous fluids, eliminating oral intake, and occasionally nasogastric suction.

People do not go to the ICU for a simple case of pancreatitis. It is a painful condition but it can be deadly. When the common duct becomes blocked from irritation, infection or a stone the enzymes of the pancreas that have been activated back up into the pancreas and begin to digest or breakdown the pancreas. It is painful, because you are being eaten by activated enzymes that have not made it to the intestinal tract, were they are harmless to people and break down food. If the enzymes have eaten through the pancreas into a vessel then he may bleed a lot and be put into the unit. If the enzymes have eaten into the liver, diaphragm or the lungs; he may go into some type of liver dysfunction, or respiratory emergency. Depending on the severity of it, it may kill you 10-15% of the time or leave you with a lot of scar tissue internally. Regardless of the severity it is very painful. So much so it will cause one to vomit. When is the last time you were in so much pain you vomited?

Regardless of the degree of his pancreatitis, he is in for a miserable few days. Being in the ICU makes me think he is either in bad shape, something else is going on, precious is being pampered or he is not really in the ICU.

The mind-body connection is an odd thing. People were kidding about it being "too much gall" or "bile," but you know, I wonder. I've certainly heard of very high-maintenance people who've never had a drink or drug in their lives but still had all kinds of problems one would expect to find in people who took more chemicals--Harlan Ellison is/was one, I think (very angry guy). The body produces its own chemistry. Who knows what's been leaking through Ashcroft's system?

That said, if they did find out he's been getting into the hooch after all, it wouldn't exactly stun me with surprise either.

family history is more important. I lost my gall bladder at age 16. My sister was 19. My mother was 23. My grandmother, 35. My great-aunt, not quite 40. My mother's a triplet, and her sister was late-twenties and her brother, late-thirties, when they lost theirs. And so on and so forth.

Our gall bladder doc (who has removed them all, except for Grandma's, LOL) says that the risk factors you've listed are all outweighed by family history. :shrug:

And now, being human, he's going through some physical pain. I have no compunctions at all about having little or no compassion for Ashcroft. Yeah, everybody goes through pain sooner or later. Some people learn to be more compassionate from the experience. My impression of Ashcroft is that if he was gonna learn, he would've long ago. He's hurting lots and lots of people. The world would be a better place without him, certainly in that place of power. But hey, I'm a reasonable woman: if it just meant he was too ill to continue his job, that'd work for me. If he *does* die, well: he believes he's going to heaven, right? Presumably he'd be happier there, he seems to have such little love for the other inhabitants of this world.

Background: Pancreatitis is an inflammatory process in which pancreatic enzymes autodigest the gland.

The gland can sometimes heal without any impairment of function or any morphologic changes. This process is known as acute pancreatitis. It can recur intermittently, contributing to the functional and morphologic loss of the gland. Recurrent attacks are referred to as chronic pancreatitis. Both forms of pancreatitis are present in the ED with acute clinical findings.


Pathophysiology: Because the pancreas is located in the retroperitoneal space with no capsule, inflammation can spread easily. In acute pancreatitis, parenchymal edema and peripancreatic fat necrosis occur first. This process is known as acute edematous pancreatitis.

When necrosis involves the parenchyma, accompanied by hemorrhage and dysfunction of the gland, the inflammation evolves into hemorrhagic or necrotizing pancreatitis.

Pseudocysts and pancreatic abscesses can result from necrotizing pancreatitis because of enzymes being walled off by granulation tissue (ie, pseudocyst formation) or bacterial seeding of pancreatic or peripancreatic tissue (ie, pancreatic abscess formation). An ultrasound or, preferably, a CT scan can be used detect both.

The inflammatory process can cause systemic effects because of the presence of cytokines, such as bradykinins and phospholipase A. These cytokines may cause vasodilation, increase in vascular permeability, pain, and leukocyte accumulation in the vessel walls. Fat necrosis may cause hypocalcemia. Pancreatic B cell injury may lead to hyperglycemia.


Frequency:


In the US: Annual incidence of acute pancreatitis is 19.5 per 100,000 population and chronic pancreatitis is 8.3 per 100,000 population per year.
Mortality/Morbidity:

Although acute pancreatitis should be noted, chronic pancreatitis has a more severe presentation as episodes recur.
Acute respiratory distress syndrome (ARDS), acute renal failure, cardiac depression, hemorrhage, and hypotensive shock all may be systemic manifestations of acute pancreatitis in its most severe form.
Race: Annual incidence of acute pancreatitis in Native American persons is 4 per 100,000 population, in white persons is 5.7 per 100,000 population, and in black persons is 20.7 per 100,000 population.

Sex: No predilection exists.

Age: The risk for African American persons aged 35-64 years is 10 times higher than for any other group. African American persons are at higher risk than white persons in that same age group. CLINICAL Section 3 of 9

Physical:

Tachycardia

Abdominal tenderness, distension, guarding, and rigidity
Mild jaundice
Diminished or absent bowel sounds
Because of contiguous spread of inflammation (effusion) from the pancreas, lung auscultation may reveal basilar rales, especially in the left lung.
Occasionally, in the extremities, muscular spasm may be noted secondary to hypocalcemia.
Severe cases may have a Grey Turner sign (ie, bluish discoloration of the flanks) and Cullen sign (ie, bluish discoloration of the periumbilical area) caused by the retroperitoneal leak of blood from the pancreas in hemorrhagic pancreatitis.
Causes:

The major causes are long-standing alcohol consumption and biliary stone disease.
In developed countries, the most common cause of acute pancreatitis is alcohol abuse.

On the cellular level, ethanol leads to intracellular accumulation of digestive enzymes and their premature activation and release.

On the ductal level, ethanol increases the permeability of ductules, which allow enzymes to reach the parenchyma, resulting in pancreatic damage.

Ethanol increases the protein content of the pancreatic juice and decreases bicarbonate levels and trypsin inhibitor concentrations. This leads to the formation of protein plugs that block the pancreatic outflow and obstruction.
Another major cause of acute pancreatitis is biliary stone disease (eg, cholelithiasis, choledocholithiasis). A biliary stone may lodge in the pancreatic duct or ampulla of Vater and obstruct the pancreatic duct, leading to extravasation of enzymes into the parenchyma.
Minor causes of acute pancreatitis
Medications, including azathioprine, corticosteroids, sulfonamides, thiazides, furosemides, NSAIDs, mercaptopurine, methyldopa, and tetracyclines
Endoscopic retrograde cholangiopancreatography (ERCP)
Hypertriglyceridemia (When the triglyceride (TG) level exceeds 1000 mg/U, an episode of pancreatitis is more likely.)
Peptic ulcer disease
Abdominal or cardiopulmonary bypass surgery, which may insult the gland by ischemia
Trauma to the abdomen or back, resulting in sudden compression of the gland against the spine posteriorly
Carcinoma of the pancreas, which may lead to pancreatic outflow obstruction
Viral infections, including mumps, Coxsackievirus, cytomegalovirus (CMV), hepatitis virus, Epstein-Barr virus (EBV), and rubella
Bacterial infections, such as mycoplasma
Intestinal parasites, such as ascaris, which can block the pancreatic outflow
Pancreas divisum
Scorpion and snake bites
Vascular factors, such as ischemia or vasculitis

for intensive care to be required my mother had gallstones and it was just snip snip and out the door. OTOH my grandfather died of Pancreatic Cancer. It was a horrendously painful and slow agonizing death.

My wife had it. I don't know the medical terminology but when gallstones block the common duct for the gall bladder and pancreas everything backs up into the pancreas and it stops functioning properly. My wife's skin and eyes turned yellow. They removed her gallbladder and inserted an abdominal drain. She was very sick and in tremendous pain for about a week but made a full recovery. What Rummy has is not uncommon for gall bladder patients.

caused by my taking prednisone for muscle/joint problems. Prednisone causes depression of the immune system. My pancreas was the part of me that suffered.

By the way, pancreatitis feels like you have a beach ball inside you, fully inflated, and you are sure it will explode and spray all your guts out. The best analogy I can come up with is the alien in the movie "Alien." That's what it feels like.

163. I had a bacterial infection of the gall bladder
that caused it to perforate. I collapsed from the fever/insult, had to have a course of serious IV antibiotics and emergency surgery. It took me six months to get to the point I could work full time again...
no health insurance... 60K bill...

I am only here because my sweetheart found me unconscious.

He is going to be going through pain and fatigue unlike anything he has had before. He may not live. That would be sad, because he has a date with Leavenworth.

He has illegally ignored fundamental constitutional protections such as the ones enumerated in the Fourth, Fifth and Sixth Amendments to the United States Consitution, and the Due Process Clause of the Fourteenth Amendment. I do not know what criminal laws he has violated, but I'm certain that he has probably violated those that are meant to protect against illegal search and seizure as a minimum.

He is in ICU for pain control.... and for other reasons they do not feel like telling us about.

Clearly this onslaught is from God, since Ashcroft has never touched alcohol. Also, check the dictionary for 'bilious.' As Ashcroft is, so has he always been.

144. I had gallstones once, he has my sympathy for that reason only.
It is terrible pain, worse than anything I've ever experienced, even a dental absess. Everything you eat makes you sick, the pain can keep going for hours, and only painkillers and/or surgery makes it stop. It sounds like his situation is complicated by some kind of infection, which probably makes it even more painful than mine was.

The up side is that you can now have your gallbladder removed using a scope and it's a lot easier to recover from it.

I worked in an ICU-CCU for six years. No case of pancreatitis would have been assigned there unless 1) things went very badly in surgery and the patient ended up on a ventilator or 2) some other process was present, such as cardiac trouble.

God and/or karma doesn't seem to do a real great job of separating the good from the bad, as far as that one goes. I guess I don't wish him pain, not that it really matters what I wish for him anyway. Once in a while people come out of these conditions a little wiser, with a little more empathy for others, but I wouldn't be money on it happening here. It would be nice if this did happen, especially in regards to medical marijuana.

I hope he doesn't use this as an inspiration to add another verse to "When the Eagle Flies" or whatever that horrible doggerel he sang was.

Grasswire is correct. The ICU status is significant and means he is in extremely bad shape. From the "Merck Manual"

The need to treat severe acute pancreatitis in an ICU can frequently be determined on hospital day 1 by any of the following danger signals: hypotension, oliguria, hypoxemia, or hemoconcentration (ie, Hct > 50%, indicating severe third space losses).

Pancreatitis is serious business.

I wonder if he has more or less privacy in the ICU. They may be able to control media coverage better and give him more privacy in ICU, although he may not have as much privacy compared to others there. Of course the others that are in ICU are probably too sick to notice him.

I guess we'll know more when they announce the next level of intervention.

There are probably untraceable substances which could cause the all-important pancreas to malfunction.

Had a girlfriend who's father's eyes looked a little yellow one day. He went to the hospital and was diagnosed with pancreatic cancer. He was dead in about 6 weeks.
