He's destroying (nearly?) all safety nets for women and children.

What kind of man doesn't take care of the women and children?

What kind of leader allows the future of his own country to languish in poverty, poor health and inadequate education?

Women and the children they bear are the future of this country. This man cannot see fit to nurture his own or the country's future.

By the way, what is wrong with the bush family and their teeth? Both shrub and poppy have ugh-ly teeth - worn down nubs of corn cob teeth. With their wealth and the current state of dentistry there's no excuse.

Whatever the reason for his smile, it's real. You can tell that by checking the edge of his eyes (away from the nose). It's crinkled. Only a genuine smile caused by an emotional response can create that effect. A fake smile, in most cases, do not have those folds because you are consciously making just your mouth muscles work.
