in a locked down inpatient facility for people with severe personality disorders. They would, of course, be removed from all family and social responsibilities for the duration of therapy, which would probably be for the rest of their lives.

They would be treated using "responsibility therapy" which allows the patient to be confronted with the effects of his actions on other people and to learn to take responsibility for them.
