Where is 40 coming from?
It may be realistic but...

BBC: More than 30 die in London blasts

CBC: At least 33 die as 4 blasts hit London

Al Jazeera: At least 33 people have been killed

CNN: 33 killed in London attacks

LA Times: Wave of Bombings in London Kill 33

NY Times: Four Blasts in London Kill at Least 33

3. Who really knows right now?
They are going to have to get all the wreckage cleaned up and FIND everyone and then check on the patients in hospitals before we know the total number of lives lost. :-( It's too soon to tell.

4. Earlier reports sourced the '40' to the ambulance service
It's possible that the numbers for the tube bombs have come vis the British Transport Police, who police the tube and overground trains in the UK. The bus would have been dealt with by the Metropolitan Police, I imagine, and perhaps they haven't collected an overall figure (deaths in the street would be harder to keep track of too, I guess).

5. The British Police press conference

had only 33 from the underground... BUT, the gentleman added
that there were fatalities of the double decker bus bombing,
and he didn't have a count. 7 would be a conservative number.

Plus, with the 100's injured, many severely, it's likely that
the final toll will go higher.
