I don't remember any other president coming off so badly in photos. What gives? Drugs/medication of some kind?

8. Part of the reason is editorial

The shot is from under his face, looking up his nose. No one looks particularly good from that angle. Unflattering photos are a subtle form of delivering the message that you disapprove of a politician. The number of unflattering photos of Bush has skyrocketed recently. I take that as a good sign.

That said, Bush is looking haggard and that persistent mouth droop has yet to be explained.
