The overall picture is from the Tweety interview. And there was a Bush-Cheney guy there, behind Dean laughing --- he asked Dean a question, and then removed a top t-shirt revealing the Bush-Cheney one. Having said that, I am not sure this isn't a slightly doctored version --- it does look a bit funny.

I use Photoshop every day for a living. The foreground/background sharpness looks faked. The shadow from Deans elbow on the dorkwad's shirt looks unreal. The perspective is off too. Deans chair would have had to been on that kid's foot for him to be that close and for the shadow to fall on him like that. Also the lighting is wrong. The lighting on Dean is from directly above. The lighting on the kid and on the audience is not. If they were that close, the shadows would have fallen differently on the laughing girls in the audience.

I've been doing photomanip for years.

The background fuzziness vs the foreground, is a result of the photographer's method.

Zoom that puppy up to 5x and check out the pixels. There are no abnormalities, nary a one.

P-Shop's a wonderful tool--I make my living using it--but it can't work miracles. Always check shadows. The lighting here is consistent w/the rest of the pic (the shadow of the head of the woman just behind Dean is cast on the woman next to her at the same angle Dean's shadow falls on the Bush/Cheney twit). Note that the shadow of the guy's chin is consistent with Dean's shadow on him as well. And there are other details that make this an unlikely fake: the parts of the guy visible between the posts holding up the arm of Dean's chair, etc.

I think he just looks weird because of depth-of-field effects. Dean is in focus, the women behind are out of focus, but the guy is in-between. I think your eye is subconsciously comparing him against the blurrier women behind and registering that he kind of pops out. He's just sitting closer to Dean, is why.

Plus he has that freeper look--that little John Hinckley sparkle they all seem to have, ready to dissolve into complete fanatical zealotry at any moment and lay waste to whatever is at hand....
