It's Eric Clapton's tribute to Italian neo-realism. He was primarily interested in Fellini's work & was simply using cocaine as a metaphor.

(John Cale was the Welsh violist with the Velvet Underground, and in spite of a long subsequent solo recording career, he was not JJ Cale, who wrote "Cocaine.")
