Help needed with sign up form for my hubby. What is the difference between USER NAME and NAME?

Username (IS THAT HIS ACTUAL NAME? OR THE CHOSEN DU SCREEN NAME?)
Username is case-insensitive.
It must only contain alphanumeric characters, underscore, or blank space.
It must not be longer than 30 characters.

Name (OR MAYBE THIS IS WHERE HIS REGULAR NAME GOES?)
It must only contain alphanumeric characters, underscore, or blank space.
It must not be longer than 50 characters.

EMail Address (I understand this one.)
It must not be longer than 50 characters.

NOTICE: Certain free email providers (Yahoo!, Hotmail, GMail, etc) will require your account to wait 24 hours before activation.

I don't recall I was this confused when I signed up...

Second question: Can you have two accounts on the same computer?

Third question: Should I contact the admins on this?
