For every success story (LOTR: Return of the King), there are a slew of third installments noted for ushering in a descent in quality. Which was the worst?

Hands down, even worse than Godfather III, if you can believe that.
The hokey 3D effects made it even worse.

The first two set the bar too high.
Of those mentioned in the OP and poll, the Godfather one is the only one I've seen. I didn't think Godfather III was too terribly good, either.

The first two took my breath away. The third felt like it was trying to strike too much of a pathos chord.
I went with The Godfather: Part III, though in all fairness I hear Coppola did it because of financial woes.

It was supposed to explain everything but it explained nothing and made things even more confusing. I absolutely loved the original Matrix but after the last one my opinion of the whole franchise soured.

19. Smokey and the Bandit 3 |
|
The first is a classic. The second was passable. The third was a crime against humanity. Just awful. It ruined the franchise. It was so bad it made me appreciate Burt Reynolds' skills as an actor.
