My daughter started with mixes of NPH and R ("regular" insulin in those days). She had to mix some, and some were just R, and she had to eat on a strict schedule with a certain number of carbs per meal/snack.
Eventually, she went to Lantus and Humalog. The Lantus was great...just one injection every 24 hours to handle the basil. Then she would inject Humalog whenever she ate anything, at a fixed unit/carb of food ratio. She was able to vary when and how much she ate, and her control improved significantly.
4 years ago, she switched to the infusion pump (she uses the Animas). It's actually more work - she tests her BG more often, but the level of control is amazing. It is now rare to see her BGs outside of 70 - 130.
As far as being low with high ketones, the secret there is normally back to a normal insulin schedule with LOTS (and lots) of fluids and frequent ketone tests. Be sure to talk to her Dr/nurse while this is going on - they always know best. If her endo isn't good about returning messages, get another, even if not pediatric. Almost all issues can be handled over the phone IF the endo is responsive.
