The state exit poll questionnaires are available in a 102 page pdf file at:

It takes awhile to download though, so you may want to go to the main website and click on the link from there:

Also, the raw exit poll data, codebooks, and questionnaires will all be available from the Roper Center on CD for $99 in a week or so.
