In all the states where it is legal, the laws require a doctor's recommendation, not a prescription. That's to avoid the DEA jerking the licenses of doctors who would prescribe it. DEA controls the licensing of the ability to prescribe, but it doesn't control free speech, so doctors can "recommend" medical marijuana in lieu of a prescription.
California is the only state with a dispensary system, which is basically medical marijuana stores. If you have your recommendation, and, depending on the locality, a medical marijuana registration card, you just go to the dispensary and buy some.
It is a bit of a racket in California. Every city of any size has clinics that appear to specialize in granting medical marijuana recommendations. Places like Medi-Cannabis. Of course, when you have to go a high priest to get your psychoactives, that'll happen. But I get the sense these clinics will write a recommendation for about anything. Feeling blue? Anxious? Nonspecific pain back?
In California and all the other medical marijuana states, patients can grow their own or have a "caregiver" grow for them. The limits vary from state to state, but are reasonable for personal use.
