It's a great little gizmo, but the pickup is a little finicky. Still, it's great to be able to do the job one handed.
They're great inventions.

Feel for your radial pulse below your thumb on the inside of your wrist. That's where you need the pickup. Just strap it on, put your wrist at chest height, and hit the button. Mine hesitates for about 5 seconds and then the little compressor kicks in.
There's really not much more to know.
