How do you talk to your boss about your screwups?

How do you talk to your boss about your screwups?
We've been swamped at my job the past couple of months, and I've been feeling overwhelmed and haven't been meeting my targets.

My supervisor has been too busy to give me last month's review, but I've been anxious about my performance and I want to get back on a good footing with her, because I need her in my corner to keep this job. Would it be a good idea to go to her directly to address things, and, if so, what's the best way to handle the approach? Thanks in advance for any advice.

1. this was just on John Tesh this week
he said having a conversation about the job needs to be separate from the performance. If you truly have been swamped at work, the boss probably hasn't had time to notice either. Just start doing good. I used to shoot myself in the foot just before performance reviews by telling my boss the things I had not done right. Instead tell him the things you have DONE right. Just pepper him with them , here and there
