Bush checks off "NO" for Vietnam service, got a link?

I had a link or copy somewhere of the piece of paper where Chimpy checked off NO to volunteering for Vietnam service. The following is posted on a military board I frequent and I want to clean his clock. Do any DU'ers have a link to this document by chance? Thanks.

President Bush flew in the ANG I believe and it was not that he didn't try to get into combat, inspite of what people would have you believe. The man has courage. I work with a man that flew with him.

5. While you're at it, ask him how to contact the man he works with.

He seems to be the ONLY one who has ever admitted he flew with the Shrub! Rare breed indeed. All of us have wondered what ever happened to the rest of his air wing, since even during all the dustup over his AWOL escapades, nobody EVER came forward to say, I flew with him, and here's what happened!
