Never happen. Why? Our good offshoring buddies downwind. This is pure sabre rattling. And it makes America look like fools and madmen. So expect conventional airstrikes for months and a few 100,000 innocents dead. No big deal.

The fact that we would lose the lives of the thousands of service men in Iraq if GWB nuked Iran DOES NOT FACTOR INTO THESE MADMEN'S decision making in any way, shape or form!

Only reason Iran isn't currently a sea of glass is the corporate oil interests can't figure out if they will or won't benefit from this action!

Your argument makes sense for rational people.
However, I am not certain at all that Bush would care if hundreds of thousands of cubic meters of earth were converted into radioactive fallout and spread mostly across what he and his wacko supporters see as an Islamofascist leaning region of the world.
As irrational as below surface detonating nukes would be, I am not at all convinced he wouldn't see that as his being a tool of divine retribution.
