OSHA fines two companies for fatal ammonia leak

SWANSEA, SC (WIS) - Months after an accidental ammonia leak killed a woman in Swansea, the state Office of Occupational Safety and Health issued nine safety violations against a company involved in the case.

Authorities with OSHA conducted an inspection of Tanner Industries and Werner Transportation Services and say seven of those violations are serious.

Among those serious violations, the report cites Tanner for not having an alarm system for employees, not providing proper training for using hazardous materials and not using a proper hose for the ammonia.

Werner Transportation Services faces four separate citations. Three of them are centered on the proper use of a respirator when it's needed. The fourth also has to do with a hose used for the ammonia.

I've worked with Ammonia and very simple well know precautions can prevent these types of accidents.
