Iowa fines BPI $1 million for fatal ammonia leak
State fines BPI $1 million for fatal ammonia leak

State regulators have fined Beef Products Incorporated more than $1 million after an ammonia leak at its Waterloo plant killed a worker.

The Iowa Division of Labor Services also issued $88,500 in fines for Modern Engineering and Piping. The company was working on the ammonia system when the leak occurred August 31st.

Both companies have 15 working days to challenge the citations.

The state opened its investigation after the leak killed 44-year-old Elizabeth Myers of La Porte City. She was 1 of several workers at the plant who were injured when a Beef Products employee opened a valve that wasn't locked, causing the ammonia to be released. Myers died a few days after the incident.

State inspectors found 34 violations, many directly related to the fatal accident.
