In the CA Primary - Ralph Nader is on the ballot for two different parties
Both the Green Party and the Peace and Freedom Party have him listed.

4. No, I was planning on going to my neighbor's garage (that just kills me).
We got the sample ballot and it had every party listed.

6. I could be wrong, but maybe because you are in a different county
This one seems to have came from the Orange County Registar of Voters.

11. I don't know.
I do know that I will be voting in the Democratic Primary. So those other ballots are really meaningless to me anyway.

16. That should be illegal. A candidate's name should only appear on a ballot once.
Unless he's running for several positions, which should also be illegal.
