1. Always look both ways before cross RR tracks.
I would think there might have been some noise too.

Stupid people didn't stop and look both ways.
It takes up to a mile to stop a train, even the engine by it's self takes hundreds of feet to stop.

Someone wasn't paying attention and got run over.

It is just stupid trying to beat the train at the crossing.
