Reporting political activity by a religious organization??

Awhile back there was a link posted to an online form to report possible violations by a church/religious organization to the IRS. I have searched (and Googled) but can't seem to find the link. Anyone have it handy?

3. It Doesn't Matter

I reported a church here in 2004 complete with sending pamphlets to the IRS. The pamphlets told the congregation how to vote and had the churches name printed on them. Didn't even get an acknowledgement and the church is still doing the same things.
