Normally, there would be poured cement surrounding the ring demarcating one diameter of pipe from the next deeper (and smaller) section of pipe.

Instead, there may (emphasis - may - they're still not sure) be only a section of cracked rock or gravel - in other words, it's not a true seal as cement would provide. That may be how gas got into the wellbore and shot up the pipe, causing the accident in the first place.
