I think it was a deliberate decision NOT to call the president "Your Honor" or "Your Excellency" or any other exceptional title.

The United States of America has has ALWAYS tried to distinguish itself from monarchies and phony despots who endow their leaders with "supernatural" powers.

I think "Mr." is just fine.
