I'm more worried about underpaid, overworked, under appreciated airline employees, particularly in the maintenance shop.

And don't forget counterfeit parts... I think '60 Minutes' did something on this a couple of decades ago...

1. Things to improve safety recommended by NTSB and never implemented by the airline industry

The list is huge of various recommendations the NTSB has discovered by investigating accidents and made recommendations to increase safety of airflight. Many have never been implemented, even though many would not cost very much to put into effect.
