4. The way I read it was....
the ones that have already been voted out of committee have to be voted out of committee again - but I don't know if they have to actually have a hearing again.

From HuffPo:

*** As part of the renomination process each of these nominees will once more have to be confirmed by the Senate Judiciary Committee. According to the committee's website, that process will begin Thursday. ***

The ones that have already had a hearing in the past such as Goodwin Liu and any others, could be brought in front of the committee to be asked just 'a couple' of questions as a group and then be voted upon.
There's no reason to start the whole hearing process from scratch.
There have been hearings where they had four or five nominees all sitting at one time together in front of the judiciary committee being asked questions (for a very short period of time), so I do not see any reason why it can't be done in this manner regarding the folks that have been renominated ;)
